Recent research has supported a peripheral action as well.54 Most opioids familiar to clinicians are straight opioid agonists or drugs with direct affinity for opioid receptors capable of binding with these receptors causing changes in the cell which stimulate physiological activity. The pure agonists have no apparent ceiling effect for analgesia. As the dose is raised, the analgesic effect increases in a log-linear function until analgesia is achieved or dose-limiting adverse effects occur. Antagonists such as naloxone interfere with the action of agonists blocking their effects. Clinically the antagonists are used to treat overdoses of opioids. It is important to be aware that agents with mixed agonist-antagonist and partial agonist action are available. The prototype agonist-antagonist is pentazocine Talwin ; . Pentazocine has agonist effects at kappa receptors and weak mu antagonist action.
